they clearly went through the express lane which in those days was almost always collocated with the office...seemed more like a retinue/cast of thousands than the normal for a suburban grocery store

Chicago--perhaps Matteson or other south suburb, I'd guess. Store dated from 1974 or so when Venture came into Chicago from St. Louis--they didn't partner in St. Louis with A&P (worked with Schnucks to put in Venture Foods) but this was A&P's last gasp for some real estate in Chicago suburba...

Were there any A&P stores left in Maine in the 1990s? I spent a couple weeks in Bangor for work, and the off weekend took a drive out to New Brunswick, and vaguely recall one east of there which was a very ramshackle looking building (didn't seem to be a Centennial--was more like a general store...

That's correct. That was the K-SO. About a mile south on Old Des Peres Road is an osteopathic hospital, so this is a reasonable location for a orthopedic practice/store.
